Ellis Pain Center accepts patients by referral ONLY.

We are dedicated in treating patients who qualify for pain management treatment. You will need your current treating physician to fill out and send us our referral form.

After your referral and the necessary records are sent to our office, our Referral Coordinator will contact you to schedule your initial evaluation. To help streamline the process, please download our new patient packet and have these documents completed before arriving at your initial appointment.

Please note, if you have been in pain management previously, we do ask that those records are made available to our providers before your first appointment. Please download and fill out our information release form to initiate a records request with your previous provider.
